Interventional pain physician · St. Louis

He spent a surgical year at the busiest trauma service in Illinois, and twenty years being wrong about what makes pain persist.

Dr. Gurpreet Singh Padda, MD, MBA, MHP is the medical director: anesthesiologist and interventional pain physician, diplomate in addiction medicine and in obesity medicine, licensed in Missouri and Illinois. What matters for a case is narrower than a CV — it is the method, and whether it is applied the same way when nobody is litigating.

Training

Biology and medicine at the University of Missouri–Kansas City in a combined program from 1982 to 1988. A general surgery year at Cook County Hospital with the University of Illinois at Chicago, then anesthesia at Illinois Masonic, finishing anesthesia at Washington University and Barnes Hospital in St. Louis from 1990 to 1992, with pediatric anesthesia at St. Louis Children’s and chief resident in the final year. An MBA in international business at Saint Louis University in 1998, with certification as a physician executive the same year.

The surgical year is the part that shows up in the reports. He reads a spine the way someone who has opened one reads it, which is also why he will say when an operation is the right answer and will not pretend an injection is one.

The correction that shaped the method

For about twenty years he told pre-diabetic patients their A1c was not too bad, injected their spines and sent them home. The injections worked for a while. The pain came back. The numbers he had called not too bad kept moving in one direction.

The correction came from physiology rather than from a conference. Hyperinsulinemia and metabolic inflammation sensitize nerves, compromise the microcirculation a healing disc or joint depends on, and keep the immune response around an injury switched on. A patient with high fasting insulin does not heal on the schedule a trial of healthy volunteers predicts.

That is why this practice measures the terrain in every patient, litigating or not — and it is the answer to the cross-examination asking why a pain physician drew metabolic labs. See the panel.

What that means for the file

A diagnosis before a treatment, a test chosen because a question was raised, and a record that shows the reasoning. Findings recorded whichever way they come out.

This practice is not aligned to a side. Its obligation runs to the patient’s health and safety, which is the same reason it is not beholden to any position that would keep it from seeing what is actually there. We do not offer opinions on liability, we do not accept a referral or a review conditioned on reaching a particular conclusion, and we will say plainly when the evidence does not support the claim — whichever party was hoping otherwise.
